• The kind of the sample: sample's name and description.

• The size of the sample: number of the sample's unit and its quality whether it is measured by weight, size or height.

• The status of the sample: (distrained – non-destrained), (frozen – non-frozen) and (in an original package or not).

 Second: Customer's need:

• In case that the customer wishes to apply the complete standard, standard's title, number, year of issuance and the relevant issued amendment should be mentioned.

• In case that the customer wishes to apply the international standards, standard's title, issuing body (BSI – AFNOR...), number and date of issuance should be mentioned.  

• The product could be (tested / analyzed) according to an attached standard submitted by the customer.

• The customer could (test / analyze) his sample according to the data written on the package of the submitted sample.

• If the customer wishes to conduct a specific number of (tests / analysis), he should:

§ State the numbers of tests to be conducted.

§ Assign the types of the required tests.

§ Mention the standard's title to which he wants to appeal.

• The item and the evaluation result must be mentioned as a report.

• Conformity certificate.

• Issued in Arabic.

• Issued in English.

• Secret number.

• Date of drawing sample.

• Number of issues.

• Device number, if any.

• To be delivered directly to an authorized delegate.

• To be sent by mail according to the customer's request.

• To be sent by fax.

• To be sent by e-mail.

To fill in the fees application form to facilitate the payment procedure whether to be paid directly to EOS treasury and hand out a copy of the payment coupon to the Sample Testing Dept.

• In respect of the samples submitted through mail or any governmental bodies which have no consignment at EOS, a claiming letter should be sent by mail according to the customer's request indicating the sample's type, size, status, secret number and the value of required fees to facilitate the payment procedures whether cash by a payable check in the name of EOS.

•   For the organization which has consignment at EOS, an application form should be submitted to the accounting Dept. to deduct the amount mentioned in the form from the customer's account.

• In case of calibrating the equipment outside EOS premises, the claiming body will cover the transportation costs from and to EOS premises, if it is necessary.

• In case of carrying out tests outside EOS premises, the requesting body will cover the testing fees, in addition to 30% from this fees as an administrative fees expenses with minimum of the value decided by EOS council.

• All charges issued by EOS council should be taken into consideration in respect of conformity certificates fees and translation fees in addition to what has been mentioned in item "5".

• The ( testing / analysis / calibration ) fees on ( samples / equipment ) should be reimbursed according to EOS council resolutions.

• In case that the withdrawn samples are not conforming with the relevant standards and consequently other samples to be tested, the testing fees should be collected with 75% discount of the determined fees. If they are also not conforming, another 50% discount to be made and then 25% discount in accordance with EOS Council Decree in this regard .
